About
Over the past several decades, the need for accessible, effective palliative and geriatrics care has climbed as advances in medicine allow for more Americans to live longer with more complex health conditions. Northwell’s Division of Geriatrics and Palliative Medicine is dedicated to meeting this need through exceptional clinical care, research and education. We provide compassionate care to patients and loved ones who are suffering with pain and other symptoms or struggling with care coordination and other challenges associated with management of chronic and serious illnesses.
Humanism, mindfulness and reflection are priorities in our practice of patient- and family-centered care. To accomplish this and provide that care seamlessly, interdisciplinary teams (including physicians, nurse practitioners, social workers, pharmacists, chaplains and volunteers) are carefully coordinated across a wide range of settings, including ambulatory office, hospital, skilled nursing facilities, home and others.
For those with serious or life-threatening illnesses, we focus on prevention and treatment of pain and other symptoms. Expert geriatricians work to promote longevity, minimize illness, reduce costly and preventable hospitalizations and readmissions, and coordinate care of complex patients.
